MEET MEG
Meg O. Lukan, M.S., L.C.P.C. has been practicing in the Field of Counseling and Rehabilitation since 1991. Her mission is to provide the highest quality of care through education, support, and self-empowerment. Through a collaborative relationship, Meg helps people confront their challenges in a safe environment with the support of their therapist as a coach.
BECOMING A CLIENT
For our mutual convenience, prior to your initial appointment, you can download, fill out and sign the following documents rather than completing them at my office. If utilizing insurance, a copy of your card will be required on first visit. Please verify “in-network” benefit prior to arrival. All co-pays and deductibles are due at the time of service.
